Job Summary
We are seeking a passionate and energetic Social Worker to join our dynamic team. In this role, you will collaborate closely with residents, families, and care teams to deliver comprehensive psychosocial support. Your proactive approach will help address complex emotional, social, and mental health needs, fostering a supportive environment that promotes dignity and well-being for vulnerable elderly populations.
Duties
- Conduct thorough psychosocial assessments of residents to identify individual needs and develop personalized care plans
- Provide counseling and emotional support to residents and their families during challenging times such as loss, grief, or mental health concerns
- Advocate for residents’ social rights and facilitate access to community resources and support services
- Collaborate with care teams to implement holistic approaches that enhance residents’ overall quality of life
- Lead or participate in case conferences, family meetings, and multidisciplinary team discussions
- Support residents in navigating family dynamics, sexuality issues, disease progression, and other complex social issues
Requirements
· Bachelor's or Master's degree in Social Work from an accredited institution.
· Registered member of the Ontario College of Social Workers and Social Service Workers.
· Experience in case management is essential; experience with elderly populations is an asset.
· Strong communication skills, both verbal and written, for effective interaction with residents, families, and staff.
· Ability to work independently as well as collaboratively within a team environment.
· Demonstrated commitment to ongoing learning
· Current Vulnerable Sector Check and clear 2 Step TB tests prior to start date
· Professional and Commercial Liability Insurance
- Experience in long-term care/ psychiatric / medical social work practice
- Certificate in Gerontology or Dementia Care
- MoCA Montreal – Cognitive Assessment trained and certified
- Gentle Persuasive Approach
- PIECES trained
- Palliative Care
