Full job description

Solus Support Services is currently seeking exceptional RSW/EPSW to provide individualized support to an individual with an Acquired Brain Injury (ABI) and/or psychosocial challenges living in the community of Keswick, Town of Georgina, Ontario.

This role is focused on promoting independence, functional skill development, and overall well-being. The ideal candidate will be comfortable providing personal care, assisting with activities of daily living (ADLs), driving, and supporting participation in meaningful community routines.

Shifts: Required to work Evenings and Weekends

Responsibilities

  • Provide 1:1, individualized support in the home and community.
  • Assist with personal care and activities of daily living (ADLs)
  • Provide safe transportation, including driving the individual to community activities
  • Assist with functional, behavioral, and cognitive goals as outlined in the support plan
  • Observe, monitor, and document progress
  • Collaborate with the team to adapt strategies and interventions based on client needs and progress.
  • Promote independence, well-being, and social engagement in all interactions.

Qualifications & Requirements:

  • College or University education in a relevant field: Enhanced Personal Support Worker or Rehabilitation Studies. (Credentials will be verified)
  • Has 2 years related experience supporting individuals with complex needs, including Acquired Brain Injury (ABI) or other neurological/trauma-related conditions.
  • Has experience in following life skills and community support plans
  • Strong communication, observation, and documentation skills.
  • Knowledge of rehabilitation principles and community-based interventions for brain injury.
  • Experience with personal care, ADLs, and functional skill coaching.
  • Gentle Persuasive Approaches (GPA) certification
  • Current Standard First Aid and CPR Level C (or BLS)
  • Clear Vulnerable Sector Check (within the last 6 months)
  • Valid Ontario G Driver’s License, access to a reliable vehicle
  • Availability to work evenings and weekends
