Full job description
Position Title: Transition Coordinator
Position Type: Part-time (approximately 20 hours per week to start), with opportunities for additional hours based on organizational growth and client demand.
Location: Vancouver, BC
Wage: Competitive wages start between $24 - $28 per hour based on professional experience, education, and demonstrated job-related interpersonal competencies.

As a Transition Coordinator, you play a vital role in supporting individuals with diverse abilities and their families through important life transitions. Whether preparing for adulthood, navigating changes in services, exploring employment and community opportunities, or planning for greater independence, Transition Coordinators help families create a clear path forward.

Can YOU coordinate supports across education, healthcare, employment, housing, and community services to help individuals achieve their goals and live their best life?
Can YOU facilitate meaningful conversations, planning processes, and person-centered goals that empower individuals to create the future they envision for themselves?
Duties and Responsibilities:
- Facilitate Person-Centered Planning processes, including PATHs, with individuals, families, and support networks.
- Serve as a consistent point of contact for individuals and families throughout the transition process.
- Develop, implement, monitor, and review individualized Care Plans and transition goals.
- Support individuals and families in identifying strengths, setting goals, exploring options, and making informed decisions.
- Coordinate and participate in transition planning meetings with families, schools, service providers, clinicians, and community partners.
- Connect individuals and families with relevant community resources, funding opportunities, government programs, and support services.
- Provide education and guidance regarding systems such as CLBC, PWD, post-secondary supports, employment services, housing resources, and community inclusion opportunities.
- Advocate for accessibility, inclusion, self-determination, and independence across all environments.
- Monitor progress toward transition goals and collaborate with the team to make adjustments as needed.
- Build and maintain collaborative relationships with community agencies, service providers, employers, educators, and healthcare professionals.
- Maintain accurate and timely records including session notes, Care Plans, transition plans, referrals, and progress updates using a digital client management system.
- Track service referrals, transition milestones, and goal attainment outcomes.
- Represent the organization's services in a professional and positive manner within the community.
- Ability to work effectively in a paperless, often remote environment.
- Perform other related duties as assigned.
Qualifications:
- Diploma or Bachelor's degree in Social Work, Disability Studies, Psychology, Education, Human Services, or a related field.
- Minimum one (1) year of experience supporting individuals with diverse abilities, developmental disabilities, autism, or complex support needs.
- Experience facilitating planning meetings, coordinating services, or supporting families through transitions is considered an asset.
- Knowledge of person-centered planning approaches, including PATH facilitation, is considered an asset.
- Working knowledge of community resources, government funding programs, and disability service systems including CLBC, PWD, and MCFD.
- Strong understanding of inclusion, self-advocacy, and person-centered service delivery.
- Excellent interpersonal, relationship-building, and communication skills.
- Demonstrated ability to work effectively with individuals, families, multidisciplinary teams, and community stakeholders.
- Strong administrative, organizational, and time-management skills with the ability to coordinate multiple priorities simultaneously.
- Excellent written and verbal communication skills, including professional documentation and report writing.
- Working knowledge and experience with a variety of computer software programs such as Google Workspace, Microsoft Office, internet-based platforms, and digital client management systems.
- A working email address with internet access at home.
- Non-violent Crisis Prevention Intervention training, MANDT, or related certification.
- Current Emergency First Aid and CPR.
- Class 5 driver's license with a clean driving record and a reliable vehicle.
- A clear criminal record check (as outlined by the Ministry for Children and Family Development).
