REPORTING TO: Director of Resident Operations/Compliance

DEPARTMENT: Resident Services

PERMANENT FULL-TIME

ORGANIZATION:

The Glebe Centre is a non-profit organization dedicated to providing compassionate Long-Term Care and meaningful community programs and services for adults at The Glebe Centre Abbotsford Community Programs. Inspired by a long history of excellence, we support our residents and the broader community through a continuum of care that prioritizes trust, respect, and industry best practices. The Glebe Centre serves 254 residents and over 2100 people in the vibrant Ottawa community. Our team of over 450 employees touches the lives of seniors and their families every day. POSITION PURPOSE:

The Program Facilitator enhances the quality of life for residents in long-term care by supporting meaningful engagement, social connection, and overall well-being through structured and inclusive programming.

RESPONSIBILITIES:

The Program Facilitator plans and delivers recreational and therapeutic activities, adapts programs to residents’ individual needs and abilities, encourages participation, and maintains a safe and inclusive environment. The role collaborates with the interdisciplinary care team, monitors and documents resident engagement and changes, prepares materials and activity spaces, and supports special events and family involvement. All duties are carried out in accordance with applicable Ministry of Health and Long-Term Care (MOHLTC) standards, legislation, and organizational policies, including health, safety, and infection prevention and control guidelines.

QUALIFICATIONS:

· A bachelor’s degree or College Diploma in Therapeutic Recreation training from a recognized University/College.

· Affiliation with a Professional Organization (NCTRC, Therapeutic Recreation Ontario or Activity Professionals Ontario or equivalent)

· .2 years of experience in Long-term Care with the elderly population and experience working with vulnerable populations and families.

· 2 years of documented experience working with dementia and mental health challenges.

· Strong leadership skills with an advanced knowledge of the aging process and dementia. Commitment to a multi-disciplinary team and a person-centered approach working with residents, clients, staff, families and volunteers.

· Excellent writing and problem-solving skills long with strong organizational and interpersonal skills.

The following will be considered assets:

· Well-developed computer skills: experience in electronic charting including Point Click Care and RAI-MDS

· Experience in Palliative Care
