  • Monitor key quality metrics and implement continuous improvement initiatives.
  • Resolve client concerns promptly and professionally.
  • Conduct regular care reviews and quality audits.
  • Ensure accurate and timely documentation.

Team Leadership

  • Lead, mentor, and develop Care Coordinators and clinical team members.
  • Foster a culture of accountability, collaboration, and continuous improvement.
  • Support caregiver education, coaching, and performance management.
  • Partner with Recruitment to ensure appropriate staffing and retention.

Operations

  • Collaborate with Scheduling to maximize continuity of care.
  • Monitor clinical KPIs and operational performance.
  • Assist with policy implementation and regulatory compliance.
  • Participate in leadership meetings and strategic planning.

Community & Professional Relationships

  • Build strong relationships with hospitals, physicians, community agencies, retirement residences, and referral partners.
  • Represent Home Instead professionally within the healthcare community.

Qualifications

  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO).
  • Minimum 5 years of progressive leadership experience in home care, community care, long-term care, or healthcare management.
  • Strong knowledge of Ontario healthcare regulations and home care best practices.
  • Experience leading multidisciplinary teams.
  • Excellent communication, coaching, and relationship-building skills.
  • Strong organizational and problem-solving abilities.
  • Proficiency with Microsoft Office and electronic health record/documentation systems.
