Full job description
Community Inclusion Society (Community Inc.) is seeking a Case Manager to join our team and support individuals who experience barriers to employment.
The Case Manager plays a key role in helping clients identify and overcome employment barriers, develop realistic career goals, and take meaningful steps toward employment. This position provides individualized support through needs assessments, return-to-work action planning, career exploration, job search assistance, employment maintenance, referrals, and ongoing follow-up.

Compensation
Hourly Wage: $26.45 – $28.06 per hour (commensurate with education and experience)
Hours of Work: 37.5 hours per week
Benefits: Comprehensive benefits package available after successful completion of probationary period.
Key Responsibilities
- Conduct comprehensive client intake interviews and needs assessments.
- Gather and assess clients' educational, employment, and personal histories.
- Identify employment, social, and disability-related barriers.
- Work collaboratively with clients to develop and manage Return to Work Action Plans (RTWAPs).
- Provide individualized support with:
- Resume and cover letter development
- Job search strategies
- Interview preparation
- Career exploration
- Skills development
- Employment maintenance
- Assist clients with applications for retraining programs, self-employment benefits, wage subsidies, work experience programs, and workplace accommodations.
- Connect clients with appropriate internal and external resources, programs, training opportunities, and community services.
- Maintain regular contact with clients and provide ongoing employment support.
- Develop and maintain knowledge of regional training opportunities, funding sources, and employment services.
- Facilitate NS Works workshops and information sessions as part of a team rotation.
- Participate in career fairs, job fairs, information sessions, and other community outreach activities.
- Build positive relationships with community and service partners.
- Participate in partner meetings and ongoing professional development.
- Maintain accurate and timely client documentation, case notes, and reporting in LaMPSS.
- Complete required client follow-up, including employment follow-up at key milestones.
- Travel to outlying and rural areas when required to assist clients in accessing services and supports.
- Perform other duties as assigned.
The Case Manager is expected to maintain high standards of service quality while promoting an inclusive, respectful, and client-focused environment.
The successful candidate will be expected to maintain regular client documentation and reporting, including daily LaMPSS entries, monthly reporting, and written documentation related to client funding applications. The position also includes ongoing follow-up with clients at key employment milestones, including the first day of employment, first week, first month, and subsequent follow-up periods.
Qualifications and Skills
- A bachelor's degree or post-secondary education in a social services discipline, or an acceptable combination of education, training, and experience.
- Existing Career Practitioner Certification, or willingness to obtain the required Career Practitioner Certification within the required timeframe.
- Case Management Specialization training is required, with the expectation that it will be obtained within one year of employment.
- Certification must be maintained in good standing where applicable.

The successful candidate will demonstrate:
- Experience conducting needs assessments.
- Strong client engagement and relationship-building skills.
- The ability to develop positive rapport and a strong working alliance with clients.
- Experience conducting intake interviews and interpreting documents.
- Strong communication, organization, and time-management skills.
- The ability to work independently while also contributing effectively as part of a team.
- Experience facilitating workshops, presentations, or information sessions.
- Strong knowledge of Microsoft Office Suite.
- Experience with LaMPSS is considered an asset.
- An understanding of and commitment to the Code of Ethics for Career Practitioners.
- The ability to establish and maintain healthy professional boundaries.
- Strong interpersonal and assertive communication skills.
