The South Georgian Bay Community Health Centre (SGBCHC) provides primary healthcare services with a wide range of other health promotion and community development services under one roof; focusing on the social determinants that impact an individual's health. The SGBCHC provides an integrated model of care by working in partnership with other agencies in the communities it serves. The CHC's programs and services are developed to respond to the specific and diverse needs of the community.
Job Summary
Reporting to the Operations Manager, the Registered Practical Nurse will be working with the SGBCHC care team to assist clients to receive primary care services using a holistic approach. The Registered Practical Nurse will be responsible for providing basic nursing care for individuals and families with a broad range of care needs.
Duties and Responsibilities
Clinical
- Provide a thorough nursing assessment of clients taking into consideration the client’s level of knowledge and readiness, socio-economic and cultural background.
- Provides direct primary nursing care, assessment and intervention including triage and intake assessment.
- Provide client care for in person appointments and provide virtual follow up in a timely coordinated fashion.
- Support and work with client and their multidisciplinary team.
- Use tele-diagnostic equipment and arrange follow up
- Consult with care team members when necessary for client situations beyond scope.
- Regular monitoring of clients within scope of practice and other clinical providers.
- Teach and counsel clients, their family members/care givers/ substitute decision makers in all facets of health management.
- Maintain professional competence and knowledge of current practices by participating in professional development
- Establish therapeutic relationships with clients, their family members/caregivers/SDMs in order to build trust and effectively deliver appropriate care services.
- Protect client confidentiality in accordance with PHIPA, CHC policies and standards established by the College of Nurses.
- Maintain a level of care that meets holistic and ethical standards based on the College of Nurses requirements.
- Uses sound professional judgment and acts in accordance with all CHC policies and procedures and standards of practice set forth by CNO.
- Assess the risk of exposure to, and transmission of, infectious agents in every nurse-client situation. Areas of concern include, but are not limited to, protective barriers, sharps, care of equipment, TB, and Influenza.
- Responsible for the care of sharps disposal, ordering supplies, maintenance of equipment and stocking examination rooms.
- Administer staff TB and Influenza vaccines.
- Laundry and other duties as assigned
Administration
- Order and maintain inventory of medical supplies and medical samples.
- Maintain clinical records and documents client assessment findings, goals, treatment plans and interventions in accordance with CHC policies and procedures and as per guidelines established by the College of Nurses.
- Participate in chart reviews and discussion of problem cases.
- Maintain an accurate and up-to date list of staff TB and influenza vaccines and ensure appropriate stock of staff Masks according to their mask fit tests.
- Collaborate with the Operational Manager and/or JH&SC representative to ensure the following:
- Staff immunization and mask fits are up to date according the CHC policy
- First aid kit and crash cart are checked monthly
- Autoclave and reprocessing is documented correctly and checked regularly
- Required PPE and hand sanitizer and soap is available
- Send stats related to SMDHU regarding the Naloxone, Needle Exchange Program and vaccine supplies
- Maintain daily log of vaccine fridge temperatures and report to SMDHU any Cold Chain events
- Maintain statistics and report on OTN activity
Education:
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
- OTN certification for providing care via telemedicine
- Training related to crisis intervention, trauma awareness, suicide prevention and supporting clients with mental illness an asset
Knowledge/Experience:
- 3-5 years of current Practical Nursing experience with a focus on primary care in a community setting.
- Knowledge and experience with OTN an asset.
- Computer literacy including experience with an EMR.
- Experience educating and health teaching individuals with diverse backgrounds.
- Knowledge of other community resources, treatment centres and organizations.
- Good understanding of the roles of other health care professionals and complementary therapies.
- Knowledge and experience of working with marginalized populations and populations with mental illness
