Acclaim Health is hiring for Personal Support Workers to work in the North Halton Community (Milton, Campbellville, Acton, Georgetown). These positions will require a minimum of 2 shifts per week and every other weekend (Saturday & Sunday) and statutory holiday rotation commitments.

PRIMARY FUNCTIONS:

• Provides homemaking and personal support services to individuals and families in the community and retirement home settings.
• Assist clients with their personal care and activities of daily living to clients, including bathing; grooming; dressing; mouth care; and checking skin integrity.
• Collaborates with members of the health care team, the homemaking and personal support services team and family in decision-making regarding planning.
• Provides bedside care in retirement homes as directed by the Nurse or otherwise directed by Supervisor.
• Assist with the maintenance of catheter and colostomy bags when required.
• Performs light housekeeping, including cleaning of clients personal laundry and bedding when required as part of the care plan.

POSITION REQUIREMENTS:

• Personal Support Worker Certificate or Diploma
• Valid Ontario Driver’s license (G2/G) and insurance
• Current CPR certificate
• Current TB test
• Palliative Experience is an asset
• Demonstrated ability to work independently and collaboratively as a team member
• Ability to use mechanical equipment (hoyer lifts) when required
• Good written and verbal communication and interpersonal skills
• Effective observation and listening skills
