As part of our commitment to improving patient outcomes, we are seeking a dynamic and experienced Patient Care Optimization Lead to join our team. This role is crucial in ensuring that our patient care processes are efficient, data-driven, and aligned with best practices in chronic disease management and population health.
Position Summary:
The Patient Care Optimization Lead will be responsible for enhancing patient care delivery through the effective management of patient panels, care coordination, and data-driven decision-making. This role will work closely with clinical teams to identify opportunities for process improvements, implement best practices, and support care teams in optimizing patient outcomes. The ideal candidate will have strong analytical skills, a passion for patient care, and experience in quality improvement in a healthcare setting.
Key Responsibilities:
Panel Management: Advise and support panels managers to ensure optimal patient care coordination and resource utilization. Monitor panel metrics and identify opportunities to enhance care delivery.
Quality Improvement: Support quality improvement initiatives focused on chronic disease management, patient access, and care continuity. Develop and track performance metrics to measure the impact of these initiatives.
Patient Engagement: Develop strategies to enhance patient engagement and adherence to care plans, including patient education, outreach, and follow-up.
Training and Support: Provide training and support to clinical teams on best practices in panel management, care coordination, and patient engagement. Serve as a resource for staff on optimizing care delivery processes.
Qualifications:
· Medical Office Assistant or Licensed Practice Nurse designation.
· Minimum of 3-5 years of experience in a healthcare setting, preferably in panel management, patient care coordination, or similar roles.
· Excellent communication, leadership, and interpersonal skills.
· Ability to work collaboratively with multidisciplinary teams.
· Proficiency in healthcare data systems, electronic health records (EHR), and quality improvement tools.
· Strong organizational skills with attention to detail and the ability to manage multiple priorities.
Job Types: Full-time, Permanent
Pay: $27.00-$31.00 per hour
