Emery-Keelesdale Nurse Practitioner Led Clinic (EKNPLC) offers a new model of primary health care in which Nurse Practitioners (NP) coordinate client care within a collaborative and vibrant interdisciplinary team environment. As part of our commitment to a philosophy of Primary Health Care we emphasize connecting and coordinating services within the health care system to those from other sectors to address the broader social determinants of health. The clinic operates from both a main site and a satellite site to offer services to individuals living in the North York West community.

Summary of Position

The Nurse Practitioner-Primary Health Care (NP-PHC) will work with diverse client populations across the health-illness continuum (CNO, 2011). Specifically, the NP-PHC will triage, assess, diagnose, treat, and monitor clients with a variety of acute or chronic illness and injuries. The NP-PHC functions interdependently with other EKNPLC staff to provide shared, client-centered care that emphasizes health promotion, disease prevention, and client education. The NP-PHC provides primary health care within the full scope of practice to clients at all stages of their lives. The NP-PHC will deliver care in accordance with EKNPLC policies and professional practice standards, with a focus on health education, chronic disease self-management, and illness/injury prevention for diverse clients of all ages.

Summary of Responsibilities

Client Care:

  • Provides holistic assessment of and care for clients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation;
  • Emphasis on conducting Home Visits of the Clinics vulnerable clients (i.e. frail elderly/geriatric or home-bound clients who require medication maintenance and vaccination administration)
  • Completes health histories and physical examinations using assessment tools as available and applicable to assess strengths and needs
  • Develops and executes a plan of care that considers the individual client's health status, preferences, as well as, current best practices
  • Formulates and communicates diagnoses, determines the need for and orders the necessary diagnostic tests, as per scope of practice, including labs, permitted X-Rays, ECGs and U/S and, interprets and acts on the results
  • Prescribes medications within scope of practice
  • Arranges follow-up appointments as necessary
  • Monitors the ongoing therapy of clients with chronic stable illnesses by providing pharmacologic and counseling interventions
  • Conducts examinations using an age/sex specific list of recommended preventative interventions and risk assessment
  • Provides coordinated, collaborative, and comprehensive quality care
  • Maintains ongoing client documentation, including assessment findings, treatment plans, and progress
  • Offers phone consultation/triage for acute episodic illness
  • Consistently records and documents in the client’s electronic health record (EMR)
  • Referral/Consultation/Collaboration:
  • Consults with NP colleagues and/or physician consultants as per practice guidelines and EKNPLC policies and procedures.
  • Liaises with other team members, other service providers, relevant community agencies/programs including external specialist consultation when required
  • Communicates with hospital and community staff and other inter-professional team members as required to ensure smooth transition for care

  • Critically appraises, utilizes, and generates nursing knowledge through research (both nursing-specific and/or inter-professional)
  • Engages in scholarly activities, such as presentations and/or publications
  • Continues to update clinical skills and keep abreast of best practices and relevant resources through continuing education opportunities
  • Engages in continuous critical reflection and quality assurance to improve care at both individual and team level
  • Possible program implementation, management and evaluation
  • Possibility of engaging in scholarly activities to acquire and maintain a clinical university appointment including teaching seminars or other educational forums for nursing, medicine and other allied health members, as well as precepting graduate students in nursing
  • Master’s Degree in Nursing or equivalent
  • Specialty Certificate: NP-Primary Health Care
  • Registered in good standing with the College of Nurses of Ontario (CNO) as an NP
  • Current membership in professional associations is preferred:
  • Registered Nurses Association of Ontario (RNAO)
  • Nurse Practitioners' Association of Ontario (NPAO)
  • Current Basic Cardiac Life Support (BCLS) Certification
  • A minimum of two years clinical experience as a Nurse Practitioner, preferably in community-based health care
  • TEACH certification and/or diabetes educator certification an asset
  • Strong health assessment/clinical skills
  • Geriatric care experiences an asset
  • Demonstrate use of theory and research/evidence based outcomes within own practice
  • Community health nursing in a primary care and or rural setting an asset
  • Teaching experience will be considered an asset
  • Clinical research experience and knowledge of basic research designs, measurement techniques and statistical methods is an asset
  • Ability to provide constructive feedback to peers individually and in group settings
  • Ability to work effectively with all members of the health care team
  • Ability to maintain confidentiality and impartiality
  • Exceptional organizational, problem-solving and time management skills
  • Ability to function with minimal supervision
  • Must possess a dependable, consistent and reliable work record including attendance at work
  • Comfort with scheduling and documentation in electronic health records
  • Capacity to adapt quickly to a fast paced, dynamic work environment
  • Must have working knowledge of an anti-oppression / pro-diversity framework
  • English required; ability to speak languages of the priority community an asset
