This is a temporary full-time position, for approximately 9 months.
The HNHB LTCH BSO Clinical Coach will use a team approach to provide holistic care to the residents of LTCH’s to which they are referred. Their practice and clinical approach through shall support the capacity building of LTCH staff through; coaching, mentoring and modeling strategies and interventions to support the LTCH to manage and care for residents with responsive behaviours using best practices, external supports by utilizing their clinical standards of practice in accordance with their regulating body.
Duties and Responsibilities:
1. Works collaboratively, professionally and ethically with other external providers/ outreach teams in that sub region or from a referring community or sub region in the support of the LTCH which they provide services to.
2. Works alongside, not in place of staff in the LTCH in order to;
a. assess and observe the interactions of the resident with staff/ other residents and the environment
b. obtain staff input on past interventions trialed and the application of new recommendations or strategies
c. assist with care and transfers (stand by, demonstrate, coach, mentor and model)
d. determine other residents who may be prone to or at risk to display responsive behaviours for which a referral could assist the resident and the staff
e. build relationships with LTCH staff and to coach best practices
f. assess the effectiveness of strategies in order to evaluate the readiness of a case to close
g. support and demonstrate Montessori or behaviour reduction activities at large with residents in collaboration with the LTCH staff, Therapeutic Recreation department, Therapy Department and leadership. (i.e. Montessori activities, reading circles, doll therapy, domestic activities such as dusting, sorting, towel folding, games etc.) to demonstrate their effectiveness as a staff capacity approach in the reduction of responsive behaviours, strength-based approaches, sun-downing interventions, and quality life for the residents.
3. Contributes to or conducts approved resident assessments in the development of a holistic plan of care based on the needs of the resident and the referring issue including a PIECES assessment on all new referrals.
4. Contributes to the resident’s care and services in consultation with the LTCH interdisciplinary team, the resident, their family and external providers through analysis, data collection, observation, and trialing of strategies alongside front line staff in the LTCH home.
5. Provides recommendations and strategies in an attempt to reduce, prevent or deescalate the responsive behaviours of the resident for which they are referred to or asked to consult on by the contact person or leadership of the LTCH to which they are assigned or are covering.
6. Ensures that the strategies they have recommended and / are trialing through mentoring, coaching and modelling are regularly evaluated and revised as needed and communicated to the home staff through face to face interaction, shift report, team meetings, Responsive Behaviour/ Risk meetings and documentation.
7. Opens and closes cases in accordance with established practices within BSO and in consultation with the LTCH Home.
8. Uses the training, assessment tools and resources provided including but not limited to PIECES, Cornell and GDS depression scales, pain assessments, Cohen Mansfield's, CAM and in consultation or collaboration with the homes.
9. Provides mentorship and support that is person centered and based on best practices.
10. Attends case conferences, sub region meetings and complex case meetings regarding their assigned or referred cases or LTCH’s where their schedule allows.
11. Communicates effectively with the home leadership and front-line staff to maintain professional relationships and conducts themselves accordingly.
12. Communicates absences, vacations and changes in hours to the home contact person as a professional courtesy and in order to ensure continuity of resident care.
13. Utilizes therapeutic communication by establishing a professional working relationship with respect to professional boundaries, privacy and consent with clients, residents, Substitute decision makers, PG&T, coworkers and others within the circle of care for the purpose of meeting the resident’s needs.
14. Committed to performance improvement activities including PDSAs, tests of change, working groups, ongoing education, staff meetings and training.
15. Provides on site leadership and direction to the BSO Clinical Support Worker in the assessment, application, documentation and demonstration of care strategies, interventions and recommendations.
16. Participates and supports successful resident transitions across health care sectors with the resident’s/SDM consent to ensure assessments, care plan strategies and recommendations for care are shared.
Knowledge, Skill and Experience
· Complete other duties as assigned by the Sub Region RN Manager/ designate.
· Current certificate in good standing from the College of Nurses of Ontario as a Registered Practical Nurse
· A minimum of two years of Long-Term Care Experiences
· Sound knowledge and application of practices as they relate to persons with Dementia
· Experience in Mental Health or related practices preferred
