We are here to help. Ontario’s 14 Home and Community Care Support Services organizations coordinate in-home and community-based care for thousands of patients across the province every day.

We assess patient care needs, and deliver in-home and community-based services to support your health and well-being. We also provide access and referrals to other community services, and manage Ontario’s long-term care home placement process.

We assess patient care needs, and deliver in-home and community-based services to support your health and well-being. We also provide access and referrals to other community services, and manage Ontario’s long-term care home placement process.

We collaborate with primary care providers, hospitals, Ontario Health Teams and many other health system partners to support high-quality, integrated care planning and delivery.

We help patients, their families and caregivers when they need services, support and guidance to:

> Remain safely at home with the support of health and other care professionals

> Return home from hospital and recover at home

> Find a family doctor or nurse practitioner

> Find community services that support healthy, independent living

> Transition to long-term care or supportive housing

> Die with dignity in the setting of their choice

In addition to our role and responsibilities to provide home and community care services, we are important health system partners actively collaborating and managing emergency response activities such as pandemic response and community, hospital and long-term care home evacuations needed in moments of unforeseen natural disasters such as floods and forest fires.

Meh!
Used to be a great job but now the money is not worth the workload.Good flexibility in the community, no micromanaging but to many complaints and complicated cases.Money is virtually the same since 2019 cause of bill 124

Understaffed and no management involvement
Understaffed…severely. Staff talking about how they’re all looking for new jobs. Co-workers have to beg each other online for help as managers are too busy to get involved. Training is terrible. Even the trainers get pulled off of training new staff to cover for others due to staffing issues. Newbies are often put as “floats” which means you cover for whoever is off, but aren’t trained in each p...

Not a good place to work.
Staff are not valued or supported. Management are aware of the excessive workloads due to understaffing/high caseload numbers but only care about metrics. Consequently, there is exceptionally high rate of staff burnout/turnover and little stability or ability to provide good service.

Approachable team
Positive environment, no micromanaging, approachable CC’s, TA’s and managers so far in my short time. So far so good. Busy at times as its a call center setting you have to be in the phones most of the time and should be a multitasker.

Extremely high workloads, extremely high demands that are very time sensitive, extremely poor work/life balance. No support from management, they only focused on stats.
