POSITION SUMMARY:

Under the direction of the NISA Programs & Operations Manager and in conjunction with the Canadian Mental Health Association-Sudbury/Manitoulin (CMHA) as outlined in the Contract Services Agreement provides a peer-based supported model to individuals residing in supportive residential housing. The Peer Support Worker will provide recovery oriented assistance in the residence with skill building related to activities of daily living (ADL), life skills, building relationships, homemaking and experiential learning.

DUTIES AND RESPONSIBILITIES:

Provides support to individuals to assist in their recovery:
Providing assistance and support with individuals in activities of daily living
Assisting individuals with needs identified in their care plan and ensuring it is delivered in a safe, timely and effective manner
Recognizing and respecting recovery based philosophy including individual’s right to self-determination and autonomy
Providing regular updates to all staff involved in the home on individuals and service operations

Assists in the daily functions of the shared living home:
Providing orientation to individuals
Assisting individuals with their nutritional needs
Ensuring individuals participate in household expectations
Ensuring house rules and regulations are followed.

Develops and maintains effective relations between staff and individuals residing in the home:
Maintaining awareness of individuals progress by reviewing files and other related documentation
Participating in house meetings, communication building and conflict resolution as required
Develops educational programs relevant to individual’s living in the home:
Organizing and conducting skill training with individuals
Evaluating and recording effectiveness of activities and sessions
Developing and planning of in house and community presentations
Providing community resources

Routinely monitors individual’s health:
Documenting and reporting any health related concerns identified by the individual
Responding immediately to any urgent health issues
Assists in meeting the objectives of the care/wellness plans
Administering medication and first aid as required
Providing support to individuals in achieving goals, particularly in the areas of basic life skills, social interactions, coping with mental illness, symptom management , medication education, monitoring progress and providing support when problems or crisis arise
Participating in the evaluation of program goals and objectives and making recommendations on changes to current program activities for improved service delivery

Ensures all file management and reporting functions are maintained in a comprehensive and timely manner:
Ensuring individual’s contacts are recorded appropriately and in a timely manner utilizing CRMS database program
Ensuring individual’s OCAN is up to date
Ensuring staff activities summaries are completed
Ensuring Individuals emergency contact information is updated
Ensuring that monthly, annual and other required reporting documentation are completed

Maintains up-to-date knowledge of the Mental Health act and related legislation, the structure and personnel of local social agencies, government departments and trends and developments in the community that could affect the provision of mental health services

Represents program and agency in a professional manner

Avails self of opportunities for professional development:
Bringing forward training needs and takes responsibility for seeking out relevant training opportunities
Participates in workshops and seminars as required
Attending and participating in Agency and program meetings as required
Participates in the performance review process

Works within the Policies and Procedures of the home and as staff of NISA

ESSENTIAL RESPONSIBILITIES AND DUTIES:

Carrying out all duties of the Peer Support Worker Residential Position in a moral and ethical manner
Being present at the scheduled time of the day ready for the performance of duties
Being ready emotionally and physically to perform duties
Be available to work varying shift times

QUALIFICATIONS

Lived experience within the mental health system
Familiar with the goals and objectives of NISA and CMHA
Have working knowledge of mental health service agencies, including consumer/survivor organizations in Ontario
A diploma, degree or Canadian equivalent in health or a related discipline, and/or a minimum of two years’ experience working in the social services or program planning is preferred
Completion of Wellness Recovery Action Plan (WRAP) and other Peer Support training is preferred
Completion of Standard First Aid with CPR, WHMIS, Nonviolent Crisis Intervention (NVCI), Applied Suicide Intervention Skills Training (ASIST), Workplace Accessibility and Prevention of Violence and Harassment in the Workplace Training is preferred.
Computer experience, including an ability to use Word, Excel, Publisher and email/internet technologies
Written and verbal proficiency in both official languages is considered an asset.
