Homewood Human Solutions ™ is seeking a bilingual professional to serve as a Disability Support Services Assistant in our Homewood Disability Treatment Program. This position is responsible for managing referrals, clinical file tracking and administration as well as operational support to the field based service providers.
The Homewood Disability Treatment Program (HDTP) is unique in Canada. HDTP delivers mental health and addiction treatment with a return-to-work focus for disability claimants.
The successful candidate will be located in our Guelph Office.
Responsibilities: Manage all administrative aspects of disability cases which includes:
Speaking with the patient telephonically
Coordinating field based service providers workflows
Setting up the case file in the database
Reviewing all case reports and invoices to ensure accuracy and timeliness
Distributing case reports to stakeholders on a timely basis
Scanning documents into the database
Tracking all case activities to the service standards
Ensuring data elements in the case file are correct
Providing basic IT/technical support to field based service providers
Administering telephonic survey questionnaires to patients
Qualifications:
Strong organizational and time management skills
Proficiency with Microsoft Office (Windows, Word, Excel, PowerPoint)
Excellent French and English verbal and written communication skills are required
Post Secondary Education in a related field (i.e. mental health and addictions) is preferred however a combination of customer service / administrative experience and related education will be taken into consideration
Experience in Disability Management is an asset
