Provide nursing care to clients and caregivers in the community by applying the nursing process within the framework of Quality (QM) and VON Branch philosophy, policies and procedures, College of Nursing and VON Canada standards and applicable legislation. Contribute to the fulfillment of the VON’s mission statement to develop and/or promote programs to meet community needs.
Responsibilities
Utilizes appropriate interviewing and communication skills to obtain information from individual/family
Provides complete, relevant assessment of individual, family and their environment
Accurately interprets the assessment data and formulates nursing diagnosis
Based on assessment data, establishes health care plan in collaboration with the client and primary caregiver identifying priorities, target dates and expected outcomes
Implements appropriate nursing interventions from care plan to provide safe, comprehensive nursing care based on scientific principles and standards of practice
Evaluates client's/family's responses to plan of care for the achievement of realistic client-centered outcomes and revises plan as required
Incorporates discharge planning into plan of care
Demonstrates respect for individual's choices or decisions
Utilizes adult learning principles to provide counselling, teaching, health promotion and disease prevention to individuals, families and groups
Analyzes and incorporates current research findings into clinical practice
Provides rationale for changes in nursing care
Documents nursing process clearly, concisely, accurately and appropriately, according to VON standards. Access Centre (CCAC) Case Manager
Qualifications
The ideal candidate must be immediately functional and possess the following qualifications:
Certificate of Competence as a Registered Nurse in the province of Ontario
Comprehensive knowledge of CNO standards
Minimum of one year of clinical practice preferred
For positions requiring expertise in a specific field of clinical practice (e.g. palliative care, foot care, specialized nursing procedures) additional educational preparation preferred and may be required
Demonstrated skill in observation, independent assessment, health teaching, communication and interpersonal relationship building
Demonstrated ability to problem solve independently and to function as an independent practitioner
Demonstrated ability to work collaboratively as a member of a team
Knowledge of community resources
