The Director of Care Services is a member of the senior management team responsible for oversight of client services and ensuring the provision of integrated care in accordance with the organization’s mandate and strategic plan. The scope of responsibilities includes the services provided by the Geriatric Assessment Intervention Network Client Intervention (GAIN) Community Team, Integrated Care Management Team, and our Community Services (Addictions Support, Adult Day Programs, Meals on Wheels, Home Support, Supportive Housing and Health Promotion). This position will use evidence based strategies to develop and support client centered outcomes and metrics, identify new service needs and funding opportunities, collaborate with partners to develop, plan and operate programs and liaise with various levels of government, community service agencies and health service providers to respond to community needs.
Reporting to the Executive Director, the Director of Care Services will be a compassionate, collaborative and visionary leader, with strong strategic agility, talent management, planning, change management and results-orientation competencies. The ideal candidate will have successfully led programs, people, strategy and change for not-for-profit organizations in the health care or community services sectors serving older adults and seniors. Experience partnering with funders such as the Ministry of Health – Local Health Integration Network (MOH -LHIN) and other community/health service sector providers will benefit the successful candidate to advocate on behalf of seniors. Candidates will hold a Master degree in Nursing, Social Work and/ or related field or equivalent experience and must be proficient in English. Proficiency in other languages such as Armenian, Cantonese, Greek, Mandarin, Tagalog and Tamil are desired.
The successful candidate for the Director of Care Services will have the following key deliverables: -
o Oversee and Manage programs and service teams geared to older adults and frail senior clients in accordance with the organizations’ and the MOH – LHIN strategic plans and objectives for older adults.
o Implement and Manage the Geriatric Assessment Intervention Network (GAIN) Community Team.
o Develop, Implement, Manage and Report on performance indicators and outcomes.
o Plan, Develop and Implement evidenced- based program and services to meet the needs of older adults.
o Support the organization’s Succession Plan by developing the talent within the Care Services team through mentoring, coaching and on- the- job training opportunities.
o Report on M-SAA (Multi-Sector Accountability Agreement).
