PHARMACIST, Part-time, Maternity Leave, Surrey
Reporting to the Manager, the pharmacist’s responsibilities include performing assessments of client’s medication use related to their medical condition, determining suitability of current medications and appropriateness of use, helping to plan and put in place medication plans for clients and to providing counselling and support to clients while they are undergoing medication changes. This position requires close collaboration with the team physician and liaison with the rest of the clinical team.
Duties and Responsibilities
Conducting assessments of new clients and assisting the physician with the management and monitoring of clients’ medication plans while in the program
Meeting one/one with clients to determine history of injury and nature of pain
Reviewing current use of medications by clients and history of use
Discussing with client medication reduction or maintenance of program
Provide recommendations for the team regarding medication plan for clients
Provide detailed medication plans to family Physician for continuity of care and liaise with family physician
Providing quality reporting and communication in a timely manner
Registered to practise Pharmacy with the College of Pharmacists of BC
Experience in medication management
Familiarity with Canadian guidelines for Chronic Non-Malignant pain management
Knowledge of narcotic medications
Experience in working in a clinical/multidisciplinary setting
Experience working with a chronic pain population is an asset
Completion of residency in Community Pharmacy is an asset
2 days/week with flexibility to work up to 3 days/week
Start of employment: on or about September 3, 2013, approximately 1 year maternity leave coverage with a potential for extension or for permanent/part-time status.
