Mental Health Support Network South East Ontario Corp. is currently recruiting for Contract Peer Support Workers for the Transitional Discharge Model project. The Peer Support Worker will meet with consumers in the community as well have opportunities to provide coverage in our Support Centres when a permanent part-time staff is not available for their scheduled shift. This position does not have any guaranteed hours associated with it, however when matched with someone being discharged from hospital, it is expected that you will meet with the consumer(s) once per week through a 3 hour shift. The contract runs until October 2014 with the possibility of extension. Peer Support Workers would have the opportunity to apply for permanent part time postings should any become available throughout our Network.
The transitional discharge model that is being practiced is based on the provision of therapeutic relationships which assumes that people heal in relationships and that a network of relationships (such as staff and peer relationships) provided during transitional period of discharge from hospital to community will assist in individuals recovery.
Peer Support is a non-clinical, positive, supportive and empathetic alternative that relies on the willingness to share the experiences of living with a mental illness and who are in recovery to their self-determined, self-defined mental health. Peer Support staff offer non-judgmental, practical suggestions and strategies that have moved them along their own recovery path.
