Rehabilitation Case Manager
Independent Case Management Services is looking for an experienced case manager. ICMS assists clients with a variety of injuries, impairments and disabilities such as traumatic brain injury, spinal cord injury, mental health disorders, chronic pain and/or physical injury A client-centered care model is used to assess a client’s goals and objectives and coordinate professional and community resources and equipment to meet their needs.
As an ICMS case manager it would be your role to ensure clear communication among treatment team members to increase efficiency and synergy within each client's team of medical and rehabilitation professionals, legal representatives, insurer(s) and family. Your work would allow your clients to focus more completely on their recovery by assisting with tasks that can be stressful or time consuming. This would include paperwork, transportation coordination, identifying treatment and service providers, discharge planning, and coordinating appointments. This would also include reducing client's financial stresses by facilitating access to and understanding of public and private funding options for immediate and future care and rehabilitation needs.
Initially this position will involve covering existing client case load during the owner's maternity leave. She would provide mentoring and transitional assistance as needed. After the maternity leave, the successful candidate would continue on with their own case load.
The successful candidate for this position will possess these qualifications:
Registered Occupational Therapist, Registered Nurse, (other case management qualifications and experience will be considered);
At least 2 years of Case Management experience;
Excellent interpersonal and communication skills (both written and verbal);
Strong customer service ethic and a caring, positive approach to working with clients in all stages of rehabilitation;
Familiarity with insurance industry practices particularly with respect to motor vehicle accident and catastrophic injury claims including the HCAI system, Form 1 and OCF18 and OCF6 submissions;
Access to a vehicle for work purposes, and personal office space;
Familiarity with Microsoft Office (Word and Excel);
And willingness to travel to meet clients within Ottawa and surrounding areas (most clients are within 1 hour travel from Ottawa, mileage is compensated).
