The Chatham-Kent Family Health Team (CKFHT) is a collaborative team of professionals including Physicians, NPs, Nurse Educators, Dietitians, Pharmacist, Occupational Therapist, Social Workers, Chiropodist, a Health Promoter, Certified Respiratory Educator and Diabetes Education Team. The CKFHT is located on four sites across the municipality of Chatham-Kent – Chatham, Dresden, Ridgetown , and Wallaceburg. The CKFHT currently has ~30,000 rostered patients and growing.

We are currently seeking a positive, energetic, team-oriented
SOCIAL WORKER (P/T 3 DAYS A WEEK)
FOR THE CHATHAM LOCATION
Reporting to the Executive Director your responsibilities include, but are not limited to:
-Work collaboratively with other team members to manage the needs of patients of the CKFHT
-Accept all patients referred by the CKFHT physicians, triage referrals and assign priority ratings.
-Identify, analyze and provide appropriate treatment and/or referral for patients’ mental and psychosocial health care problems.
-Assistant patients and providers to navigate the health and community service systems to identify and connect with appropriate services and supports
-Case management, including links to community services in such areas as daily living, crisis intervention, health promotion and prevention.
-Collection of data as required for statistical/reporting purposes
-Observe, collect and record patient
-Comply with established personal policies, procedures, standards and guidelines for operation
-Provide clinical leadership
-Involve patients in decisions about their own health
-Graduate degree in Social Work from a recognized university.
-Minimum 3 years combined education and experience in the human service field.
-Good understanding of relevant provincial and local legislation and government policies, procedures and practices pertaining to services provided by social workers.
