  • Provide ongoing comprehensive primary health care to clients with mental health issues, their families and the community with a strong emphasis on health promotion and disease prevention
  • Work in a multidisciplinary community mental health based care setting
  • Consult with other health professionals as required to establish a diagnosis and plan of care
  • Provide ongoing follow up and evaluation of the plan of care with the client
  • Provide care to complex clients who have multiple co-morbid conditions and walk-in clients who present with episodic illness


  • Baccalaureate in Nursing from a recognized university
  • Certificate as a Primary Care Nurse Practitioner from a recognized institution
  • Current registration in the Extended Class with the College of Nurses of Ontario
  • Knowledge and experience with therapeutic methods and practices based upon a health promotion/illness prevention model
  • Strong clinical skills
  • Interest or experience with provision of care to clients with mental health issues
  • Demonstrated ability, commitment to and knowledge of community health care
  • Demonstrated ability to work in an interdisciplinary team environment
  • Excellent interpersonal skills with clients in a diverse practice population
  • Excellent communication, decision-making, problem-solving, conflict management and time management skills
  • Proficiency in the use of computers and appropriate software applications
